Summersville Public Library

The Summersville Public Library is an official passport application acceptance facility. Customers can apply for a U.S. passport and/or passport card.

You must apply in person if any of the following are true:

• You have never been issued a U.S. passport or
• You were under age 16 when your previous passport was issued or
• Your most recent U.S. passport was issued more than 15 years ago or
• Your most recent U.S. passport was lost or stolen or
• Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change

PLEASE BRING:

1. Completed Application for a U.S. Passport, Form DS-11 (PDF)

NOTE: Do not sign application until requested to do so by authorized agent

2. Proof of Identity

Approved forms of ID:

• Government-Issued Driver’s License
• State ID card
• Previous Passport
• Certificate of Naturalization with identifiable photo
• Certificate of Citizenship with identifiable photo
• Government Employee Identification with identifiable photo
• Official U.S. military or military dependent ID
• Current (valid) foreign passport

3. Proof of U.S. Citizenship

Submit an original AND a photocopy.
Documents are submitted with the application and will be mailed back to you by the State Department.

Acceptable proof of citizenship includes:

• Naturalization Certificate
• Previous U.S. Passport (may be expired but must be undamaged)
• U.S. Birth Certificate *

4. Photo

• Must have been taken in the past six months
• Front view with a white background
• Must be 2″ x 2″ in size

NOTE: Passport applications for minors under age 16 may require additional documentation. See the U.S. Department of State website for more information.

Our passport agent is generally available to help patrons and answer questions Monday-Friday, 9 am to 5pm.

Processing Times

Routine: 6–9 weeks

Expedited (Additional $60 Fee): * 3–5 weeks

NOTE: Processing times begin the day a Passport Agency or Center receives your application, not the day you apply for a passport at a local acceptance facility.

We accept completed passport renewals, but they also may be done by mail if certain conditions are met.
